In event of a serious injury, and subject to attempt to contact the parent/carer, I/We agree to the above member receiving emergency medical treatment deemed necessary by the clubs personal present. I/We also hereby indemnity the Club against any liability for injury, loss or damage sustained in connection with the Club's activities.
